Degradable material assisted diversion (DMAD) methods for well treatment, DMAD treatment fluids, and removable plugs for DMAD in downhole operations. A slurry of solid degradable material is injected into the well, a plug of the degradable material is formed, a downhole operation is performed around the plug diverter, and the plug is then degraded for removal. Degradation triggers can be temperature or chemical reactants, with optional accelerators or retarders to provide the desired timing for plug removal. In multilayer formation DMAD fracturing, the plug isolates a completed fracture while additional layers are sequentially fractured and plugged, and then the plugs are removed for flowback from the fractured layers. In DMAD fluids, an aqueous slurry can have a solids phase including a degradable material and a fluid phase including a viscoelastic surfactant. The solids phase can be a mixture of fibers and a particulate material.

A method for performing percutaneous interbody fusion is disclosed. The method includes the steps of inserting a guide needle posteriorly to the disc space, inserting a dilator having an inner diameter slightly larger than the outer diameter of the guide needle over the guide needle to the disc space to enlarge the disc space, and successively passing a series of dilators, each having an inner diameter slightly larger than the outer diameter of the previous dilator, over the previous dilator to the disc space the gradually and incrementally increase the height of the disc space. Once the desired disc height is achieved, the guide needle and all the dilators, with the exception of the outermost dilator, are removed. An expandible intervertebral disc spacer is then passed through the remaining dilator and positioned in the disc space. The disc spacer is expanded to the required disc height, and then a bone matrix is passed through the dilator to fill the disc space. The dilator is then removed. An expandible intervertebral disc spacer is also disclosed, having a tapered bore that causes greater expansion of one end of the spacer with respect to the other. A kit for performing the percutaneous interbody fusion procedure is also disclosed.

A flexible medical intubation instrument provided for placement into an animal or human patient comprises a catheter with at least a pair of longitudinally extending lumens or channels including a sensor and/or actuator channel and a working channel. While making observations through the sensor channel, the working channel simultaneously functions as a drain or an irrigation duct, a feeding tube, or to provide a passage for the insertion of one or a succession of surgical devices such that the catheter serves as a protective artificial tract or liner as surgical devices are inserted and removed through it in succession so as to minimize tissue trauma, infection, and pain experienced by the patient. The instrument can be used in urology, as well as a visually directed nasogastric tube, as a visually directed external gastrostomy tube, or as a visually directed internal gastric tube or percutaneous endoscopic gastrostomy tube and in other applications.

Apparatus and method for connecting a conduit to a hollow organ

An apparatus and method for connecting a first conduit to the heart without the need for cardiopulmonary bypass. The first conduit may then be attached to a second conduit that has a prosthetic device interposed. The second conduit may be connected to the aorta prior to the first conduit being attached to the heart. The prosthetic device may be a prosthetic valve or a pump, for example. The apparatus of the present invention includes an implantable connector with first conduit component, a retractor expansion component, a coring component, and a pushing component. The retractor expansion component is slide-ably coupled to the coring component. The retractor expansion component serves to seat against and separate the inside apical wall of the left ventricle so that the coring component may cut cleanly through the myocardium to form a tissue plug without leaving any hanging attachments to the inside walls. By remaining seated against the inside wall, the retractor expansion component follows the tissue plug into the coring component. The surgeon applies force and rotary motion to the pushing component sufficient to cut the tissue plug and implant the prosthetic component.
