Endoscopic submucosal dissection (ESD) is an advanced surgical procedure using endoscopy to remove gastrointestinal tumors that have not entered the muscle layer. ESD may be done in the esophagus, stomach or colon. Application of endoscopic resection (ER) to gastrointestinal (GI) neoplasms is limited to lesions with no risk of nodal metastasis. Either polypectomy or endoscopic mucosal resection (EMR) is beneficial for patients because of its low level of invasiveness. However, to ensure the curative potential of these treatment modalities, accurate histopathologic assessment of the resected specimens is essential because the depth of invasion and lymphovascular infiltration of the tumor is associated with considerable risk for lymph node metastasis. For accurate assessment of the appropriateness of the therapy, en bloc resection is more desirable than piecemeal resection. For a reliable en bloc resection of GI neoplasms, a new method of ER called endoscopic submucosal dissection (ESD) has been developed.

A portion of the lever portion is configured to be inserted beneath a section of mucosal tissue having a lesion, and the lever portion is configured to be rotated or otherwise maneuvered to lift the mucosal tissue in an upward direction, thereby facilitating removal of the tissue comprising the lesion. Optionally, a surgeon may advance a needle knife through the endoscope and lever portion to further incise submucosal tissue while the lever portion is disposed beneath the lesion. If desired, a flushing fluid may be provided to a target site during the procedure.

Apparatus and methods for endoscopic resection of tissue

The present invention provides apparatus and methods for performing endoscopic mucosal resection and endoscopic submucosal dissection of tissue. In a first embodiment, a catheter having proximal and distal ends and a balloon disposed near the distal end of the catheter is provided. A portion of the distal end of the catheter is configured to be inserted beneath a section of mucosal tissue having a lesion, and the balloon is configured to be inflated to lift the mucosal tissue in an upward direction, thereby facilitating removal of the tissue comprising the lesion. Optionally, a surgeon may advance a needle knife through the catheter to further incise submucosal tissue while the balloon is in the inflated state. If desired, a flushing fluid may be provided to a target site during the procedure.

Cap for medical device

A cap (10) for a medical device includes a base member (11). The base member (11) is attached to a distal portion (2a) of an insertion part (2) of a medical device (1) having an endoscope (4). On the base member (11), two linking members (13) are swingably supported by shaft members (12). On distal ends of the linking members (13), a blade (15) is rotatably supported by coupling members (14). The blade (15) has a treatment hole (15a). A treatment part (3) that is inserted through the medical device (1) has a treatment distal portion (33). The treatment distal portion (33) is inserted through the treatment hole (15a). In an endoscopic submucosal dissection, the treatment distal portion (33) makes an incision between a submucosa (101) and mucous membrane (102). The blade (15) pushes up the mucous membrane (102) that is dissected from the submucosa (101), thereby enabling the treatment part (3) to be slipped under the mucous membrane (102).
