Endoscopic submucosal dissection (ESD) is an advanced surgical procedure using endoscopy to remove gastrointestinal tumors that have not entered the muscle layer. ESD may be done in the esophagus, stomach or colon. Application of endoscopic resection (ER) to gastrointestinal (GI) neoplasms is limited to lesions with no risk of nodal metastasis. Either polypectomy or endoscopic mucosal resection (EMR) is beneficial for patients because of its low level of invasiveness. However, to ensure the curative potential of these treatment modalities, accurate histopathologic assessment of the resected specimens is essential because the depth of invasion and lymphovascular infiltration of the tumor is associated with considerable risk for lymph node metastasis. For accurate assessment of the appropriateness of the therapy, en bloc resection is more desirable than piecemeal resection. For a reliable en bloc resection of GI neoplasms, a new method of ER called endoscopic submucosal dissection (ESD) has been developed.

The invention belongs to the fields of high polymer materials and medical equipment and relates to a submucous injection material composed of physical cross-linked hydrogel and an application thereof to preparing injection materials for endoscopic submucosal dissection. The submucous injection material consists of an amphiphilic block copolymer, a solvent which takes water as a main body, and a coloring indicator, wherein the amphiphilic block copolymer is formed by taking polyethylene glycol (PEG) as a hydrophilic block and taking degradable polyester as a hydrophobic block. The material has thermal induced gel-forming properties; when the temperature is lower than a sol-gel transition temperature, a water system presents low-viscosity solution, has good injectable property, and can be subjected to endoscopic injection to the submucosal position so as to spontaneously form in-situ physical hydrogel under the body temperature, good gel morphology is maintained during an ESD operation, the uplift height and duration of the submucous injection material are superior to those of an existing pure liquid material, and the ESD operation is smoothly performed.

Apparatus and Method for Endoscopic Submucosal Dissection

A kit and a method for delivering an injectable solution to a tissue treatment site are provided. The kit includes a housing having a chamber, a proximal portion and a distal portion. An injectable solution having a viscosity greater than about 10,000 cP is provided in the chamber. A plunger is provided in the proximal portion of the housing. The kit also includes a pressure gauge operably connected to the housing. The kit may also include a handle connected to the housing and a plunger advancing member having a plunger handle connected thereto provided separate from the housing. The kit may also include an inner shaft provided separate from the housing and having a proximal end portion configured for operably connecting with the distal portion of the housing for receiving the injectable solution therethrough and a distal end of the shaft configured for insertion in to the tissue treatment site.

The preparation method of the electric knife bit comprises the steps that 316L stainless steel and ZrO2 ceramic are selected as a shell layer material and a core layer material correspondingly; shell layer material feed and core layer material feed are obtained through mixing and pelletizing; then the shell layer material feed and the core layer material feed are sequentially injected into a mold cavity through a co-injection forming technology to obtain a product green body; and then a binder in the green body is removed through solvent degreasing and thermal degreasing technologies, and finally a structural and functional integration electric knife bit product is obtained through co-sintering densification. According to the electric knife bit, a powderco-injection forming technology is adopted, the processing efficiency is high, the production cost is low, a composite material with the excellent shell layer and core layer binding property and thestable quality is prepared, the problems such as poor product performance, low efficiency and high cost in existing technologies are solved, and the preparation method is suitable for preparing the electric knife bit for the endoscopic submucosal dissection.
