One aspect of this invention is a tongue retractor (LTR) that indirectly retracts tongue base by its implant site in the frenulum area. This simplifies the insertion, adjustment and maintenance of the device. Another aspect of this invention describes a highly localized and fully implantable LTR that is inserted into the base of tongue to stiffen lax surface mucosa or mechanically couple it to internal tongue structures. A custom, sized and fitted, integrated oral appliance is provided to treat breathing obstruction and restriction in the upper airway during sleep. Several novel features are integrated in a single device, which includes mandible repositioning to open the airway, and tongue restraint to prevent airway blockage. This device is comprised of a built-in air conduit to bypass nasal restriction and airway occlusion. In operation, the appliance is positioned in the oral cavity to firmly grip the upper and lower dentition using customized unshaped trays. Laser energy may be applied to a tissue region via an optical fiber advanced through a lumen of at least one needle. Fluid may also be infused or injected into the tissue region through at least one needle. In accordance with one embodiment of the invention, there is provided a wound drainage system for draining fluid from a wound of a patient. The system includes a drain catheter. A suction means applies suction at the drain catheter such that fluid is drawn from the wound. While drawing fluid from the wound, a controller periodically increases and decreases the application of suction at the drain catheter. This allows the tissue surrounding the wound drain to relax, thus preventing obstruction of the drain catheter.

The invention provides a solid oxide fuel cell device and a fuel cell system incorporating a plurality of the fuel devices, each device including an elongate substrate the length of which is the greatest dimension such that the elongate substrate has a coefficient of thermal expansion having only one dominant axis that is coextensive with the length. A reaction zone is provided along a first portion of the length for heating to an operating reaction temperature, and at least one cold zone is provided along a second portion of the length that remains at a low temperature below the operating reaction temperature when the reaction zone is heated. A plurality of fuel passages and oxidizer passages are provided in the elongate substrate extending from the at least one cool zone to the reaction zone, each fuel passage having an associated anode in the reaction zone, and each oxidizer passage having an associated cathode in the reaction zone positioned in opposing relation to a respective one of the associated anodes. An electrolyte is disposed between each of the opposing anodes and cathodes in the reaction zone. The system further includes the devices positioned with their first portions in a hot zone chamber and their cold zones extending outside the hot zone chamber. A heat source is coupled to the hot zone chamber to heat the reaction zones to the operating reaction temperature. A fuel supply is coupled outside the hot zone chamber to the at least one cold zones in fluid communication with the fuel passages for supplying a fuel flow into the fuel passages.

A ventilation apparatus for treating sleep apnea is provided. A ventilator controlled by a control system may deliver ventilation gas through a ventilation gas delivery circuit to a ventilation catheter and a distal tip on the ventilation catheter. One or more sensors may detect a breathing cycle and the control system may operate the ventilator in synchrony with the breathing cycle. The distal tip may deliver the ventilation gas superiorly from the transtracheal ventilation catheter towards an upper airway, inferiorly from the transtracheal ventilation catheter towards a lung, or a combination of both. The ventilation catheter may be a transtracheal catheter, a trans-oral catheter or a trans-nasal catheter.
