The invention is a transgastric method for the endoscopic partial fundoplication for the treatment of gastroesophageal reflux disease (GERD). The method makes use of an articulated endoscope, which is introduced through the mouth and esophagus into the stomach of the patient. A cutting tool is introduced through the working channel of the endoscope to cut a hole in the wall of the stomach. The endoscope is pushed through the hole and a grabbing tool is introduced through the working channel and used to grab the tissue at a location on the outer surface of the stomach and move the grabbed tissue close to the esophagus. The grabbed tissue is then stapled to the esophagus using a stapling device that is an integral part of the endoscope. If desired, after stapling the endoscope can be rotated one or more times and the procedure repeated. After the selected portions of the stomach wall have been attached to the esophagus, the endoscope is withdrawn from the body of the patient and the hole in the stomach wall is closed, preferably by means of a dedicated endoscopic stapling device especially designed for the task of closing holes in tissue.

The data from the several frames are registered to each other to provide a complete three-dimensional virtual model of the dentition. Individual tooth objects are obtained from the virtual model. A computer-interactive software program provides for treatment planning, diagnosis and appliance from the virtual tooth models. A desired occlusion for the patient is obtained from the treatment planning software. The virtual model of the desired occlusion and the virtual model of the original dentition provide a base of information for custom manufacture of an orthodontic appliance. A variety of possible appliance and appliance manufacturing systems are contemplated, including customized archwires and customized devices for placement of off-the shelf brackets on the archwires, and removable orthodontic appliances.

Treatment of tissue in sphincters, sinuses and orifices

The invention provides a method and system for ablation of body structures or tissue in a sphincter, sinus or orifice such as the rectum, colon, esophagus, vagina, penis, larynx or pharynx. In one aspect of the invention, the environment surrounding the targeted ablation region can be isolated or controlled by blocking the flow of gases or liquids using an inflatable balloon positioned immediately adjacent to the tissue that is to be ablated. In a preferred embodiment, the inflatable balloon also serves to anchor the catheter in place and prevent the catheter from being expelled from the body. The inflatable balloon also insures that locally administered drug remain in the area where most needed. In a second aspect of the invention, positive pressure is used to inflate the balloon. Inflation of balloon triggers the extension of at least one curvilinear electrode into the targeted tissue. Negative pressure deflates the air sac and helps retract the curvilinear electrodes so as to allow the catheter to be removed from the body without damaging adjacent body structures. In a third aspect of the invention, the electrodes are coupled to sensors that measure properties of the target region such as temperature and impedance. Measurement of these properties permits the use of feedback technique to control delivery of the RF energy and administration of fluids for cooling and hydrating the affected tissues. In a fourth aspect of the invention, the catheter includes an optical path that can be coupled to external viewing apparatus. In this way, the position of the electrodes in the body can be determined by fluoroscopic or fiber optic techniques.
