A device for retracting edges of an incision in a surface to form an opening including: a flexible, tubular skirt having an upper end, a lower end, and a channel therebetween; a ring connected to the lower end of the skirt for maintaining the lower end in an open configuration and defining an exit opening to the channel; and an inflatable collar connected to the skirt and surrounding the upper end. The ring is designed to fit through the incision and remain under the surface when it is oriented parallel to surface. The collar, when inflated, maintains the upper end in an open configuration and defines an entry opening to the channel. During use, the ring is inserted through the incision and the collar is inflated while remaining outside of the incision, thereby drawing the skirt against the edges of the incision and retracting the edges of the incision to form the opening. The retracting device can be included in a surgical access port, which further includes a flexible sleeve connected to at least one of the inflatable collar and the skirt, extending the channel from the exit opening of the skirt to an open end of the flexible sleeve distal to the skirt. The device can include a light source in the vicinity of the exit opening.

A protective article formed of a laminate between at least a layer of a liquid-impermeable, vapor-permeable barrier, and at least a layer of a stretchable or elastomeric, nonwoven fiber web is described. The barrier is reinforced at least on one side by the nonwoven fiber web, which remains elastic after being bonded to the barrier layer. The elasticity of both barrier and bonded fiber layers simulate the flexibility of natural or synthetic latex or other polymer films. The nonwoven fiber web includes at least about 75% of individual fibers with a length of over about 1 mm, and the fibers are substantially continuous. A second nonwoven web, a second barrier layer, or both may be attached to the exterior side of the breathable barrier. An elastomeric material coating, such as either a nature or synthetic latex or other polymers, may be applied over at least a portion of the article to provide for additional protection.

A method is provided for performing knee replacement surgery on a leg of a patient. The method includes making a cut on a posterior portion of a patella, making a cut on a proximal portion of a tibia, and making a cut on the distal portion of the femur. The cut on the patella is performed before making the cuts on the tibia and femur.

A modified trocar system to assist in surgical procedures. In one embodiment, the system is designed to assist in lifting lift a body wall in order to perform surgical procedures within the body cavity without establishing insufflation. The flanged trocar embodiment includes an extended flange forming a portion of the trocar body wall that may be inserted through the incision and deployed against either the interior surface of the body wall, or the interior surface of a protective sealing ring. A lifting mechanism forming a portion of the upper part of the flanged trocar is used in conjunction with the extended flange to enable the surgeon to draw the retained trocar upward in order to provide additional access to the body cavity under the tented body wall. In a more general embodiment of the invention, the modified trocar is directed through an existing incision and is used in an inward-to-outward manner to create a second smaller incision. The second smaller incision is designed to enable the deployment of some operating equipment or attachment device. Larger equipment may then be deployed through the larger first incision and coupled to the unit inserted into the smaller incision. In this way, a variety of manipulations may take place within the body cavity with minimal incision sizes and reduced traffic into and out of the incisions.

Polyhydroxyalkanoates (PHAs) from which pyrogen has been removed are provided for use in numerous biomedical applications. PHAs which have been chemically modified to enhance physical and/or chemical properties, for targeting or to modify biodegradability or clearance by the reticuloendothelial system (RES), are described. Methods for depyrogenating PHA polymers prepared by bacterial fermentation processes are also provided, wherein pyrogens are removed from the polymers without adversely impacting the polymers' inherent chemical structures and physical properties. PHAs with advantageous processing characteristics, including low melting points and/or solubility in non-toxic solvents, are also described. PHAs are provided which are suitable for use in in vivo applications such as in tissue coatings, stents, sutures, tubing, bone and other prostheses, bone or tissue cements, tissue regeneration devices, wound dressings, drug delivery, and for diagnostic and prophylactic uses. Properties which are selected for include degradability, elasticity, inclusion of functional groups or derivatized groups, which can in turn be used to attach targeting agents, and bioadhesion.

A first aspect of the present invention is directed to a surface treatment for elastomeric articles such as medical gloves, comprising a water-based coating formulation and antimicrobial agent(s) embraced therein in an essentially powder-free composition coating. A second aspect of the present invention is directed to a water-based coating formulation for elastomeric articles such as gloves comprising at least one non-volatile water-soluble antimicrobial agent in a controlled-release matrix comprising a blend of a hydrophilic polymer and a hydrophobic component. The controlled-release matrix/blend requirements include: compatibility with the antimicrobial agent, formation of a reservoir of antimicrobial agent, coating film flexibility and lower water-solubility than the antimicrobial agent. A third aspect of the present invention is directed to a method for treating medical gloves comprising dipping, spraying, tumbling or other means for applying to the glove surface a composition to provide medical gloves with sustainable antimicrobial activity.

A draping system provides a continuous sterile field between a patient incision area and one or more medical practitioners. The draping system has a quick release system incorporated into the drape that enables a medical practitioner to separate from the continuous sterile field without disrupting the sterile field around the practitioner or the patient. The draping system may include an abbreviated practitioner gown, a drain and an integral patient incision area or a flap for extending the continuous sterile field around to one or more additional operating room tables. Latex articles with geometrically defined surface structure providing enhanced grip characteristics in dry, wet or oily environment; and a method of making same comprising applying a polymeric coagulant coating to a smooth former surface, wherein the coating becomes tacky during drying, applying discrete coagulant particles of selected size, shape and distribution to the tacky coating to attach and protrude from the former surface with the polymeric coagulant coating, dipping the former into an aqueous latex emulsion, wherein the polymeric coagulant coating and the discrete coagulating particles destabilize the latex, thereby developing a latex layer, vulcanizing and stripping the latex article inside out, and dissolving the discrete coagulant particles in water or suitable solvents to reveal the geometrically designed texture with pre-selected size, shape and distribution of impressions providing improved dry, wet and oily surface grip by removal of a fluid boundary layer.

Disclosed herein is an elastomeric article having a hydrogel donning coating. The hydrogel donning coating is made of a copolymer of hydroxy ethyl acrylate (HEA), hydroxy ethyl methacrylate (HEMA), and acrylic acid. The hydrogel donning coating is applied to the elastomeric article using an aqueous hydrogel dipping solution containing the hydrogel donning coating copolymer. The aqueous hydrogel dipping solution demonstrates thixotropic characteristics which allows for better manufacturing process control.

A material for making elastomeric nitrile rubber articles is disclosed. In particular, the process and material formulation can produce nitrile rubber based articles, which exhibit force-strain characteristics that are comparable to those of natural rubber latex articles, while maintaining the tensile strength properties of nitrile rubber. The process includes an accelerator composition at the pre-cure stage having a dithiocarbamate, a thiazole, and a guanidine compound. The invention also includes an elastomeric nitrile rubber product made by the process, such as examination, surgical, or work gloves, which can have an exterior coating over naked rubber selected from either a silicone emulsion or oil, glycerin solution, a combined mixture of silicone and glycerin/glycerol, a fluorocarbon release agent, or a natural or synthetic hydrocarbon-based oil in amounts that do not degrade said substrate rubber material.

A system and method for implanting a wrap on a patient's heart or other organ provides a wrap having finger-engagement members typically defining loops that encompass all or a substantial portion of the fingers of a surgeon's treatment hand. The loops retain the fingers to the wrap during implant, and enable the fingers to be detached after the procedure is complete. The loops can be detachable from the wrap after the procedure is completed or can be formed to remain with the wrap.
