Surgical access port

A device for retracting edges of an incision in a surface to form an opening including: a flexible, tubular skirt having an upper end, a lower end, and a channel therebetween; a ring connected to the lower end of the skirt for maintaining the lower end in an open configuration and defining an exit opening to the channel; and an inflatable collar connected to the skirt and surrounding the upper end. The ring is designed to fit through the incision and remain under the surface when it is oriented parallel to surface. The collar, when inflated, maintains the upper end in an open configuration and defines an entry opening to the channel. During use, the ring is inserted through the incision and the collar is inflated while remaining outside of the incision, thereby drawing the skirt against the edges of the incision and retracting the edges of the incision to form the opening. The retracting device can be included in a surgical access port, which further includes a flexible sleeve connected to at least one of the inflatable collar and the skirt, extending the channel from the exit opening of the skirt to an open end of the flexible sleeve distal to the skirt. The device can include a light source in the vicinity of the exit opening.

Modified trocar and methods of use

A modified trocar system to assist in surgical procedures. In one embodiment, the system is designed to assist in lifting lift a body wall in order to perform surgical procedures within the body cavity without establishing insufflation. The flanged trocar embodiment includes an extended flange forming a portion of the trocar body wall that may be inserted through the incision and deployed against either the interior surface of the body wall, or the interior surface of a protective sealing ring. A lifting mechanism forming a portion of the upper part of the flanged trocar is used in conjunction with the extended flange to enable the surgeon to draw the retained trocar upward in order to provide additional access to the body cavity under the tented body wall. In a more general embodiment of the invention, the modified trocar is directed through an existing incision and is used in an inward-to-outward manner to create a second smaller incision. The second smaller incision is designed to enable the deployment of some operating equipment or attachment device. Larger equipment may then be deployed through the larger first incision and coupled to the unit inserted into the smaller incision. In this way, a variety of manipulations may take place within the body cavity with minimal incision sizes and reduced traffic into and out of the incisions.

Polyhydroxyalkanoates (PHAs) from which pyrogen has been removed are provided for use in numerous biomedical applications. PHAs which have been chemically modified to enhance physical and / or chemical properties, for targeting or to modify biodegradability or clearance by the reticuloendothelial system (RES), are described. Methods for depyrogenating PHA polymers prepared by bacterial fermentation processes are also provided, wherein pyrogens are removed from the polymers without adversely impacting the polymers' inherent chemical structures and physical properties. PHAs with advantageous processing characteristics, including low melting points and / or solubility in non-toxic solvents, are also described. PHAs are provided which are suitable for use in in vivo applications such as in tissue coatings, stents, sutures, tubing, bone and other prostheses, bone or tissue cements, tissue regeneration devices, wound dressings, drug delivery, and for diagnostic and prophylactic uses. Properties which are selected for include degradability, elasticity, inclusion of functional groups or derivatized groups, which can in turn be used to attach targeting agents, and bioadhesion.
