The invention relates to a microfabricated device for the rapid detection of DNA, proteins or other molecules associated with a particular disease. The devices and methods of the invention can be used for the simultaneous diagnosis of multiple diseases by detecting molecules (e.g. amounts of molecules), such as polynucleotides (e.g., DNA) or proteins (e.g., antibodies), by measuring the signal of a detectable reporter associated with hybridized polynucleotides or antigen / antibody complex. In the microfabricated device according to the invention, detection of the presence of molecules (i.e., polynucleotides, proteins, or antigen / antibody complexes) are correlated to a hybridization signal from an optically-detectable (e.g. fluorescent) reporter associated with the bound molecules. These hybridization signals can be detected by any suitable means, for example optical, and can be stored for example in a computer as a representation of the presence of a particular gene. Hybridization probes can be immobilized on a substrate that forms part of or is exposed to a channel or channels of the device that form a closed loop, for circulation of sample to actively contact complementary probes. Universal chips according to the invention can be fabricated not only with DNA but also with other molecules such as RNA, proteins, peptide nucleic acid (PNA) and polyamide molecules.

Integrated active flux microfluidic devices and methods

The invention relates to a microfabricated device for the rapid detection of DNA, proteins or other molecules associated with a particular disease. The devices and methods of the invention can be used for the simultaneous diagnosis of multiple diseases by detecting molecules (e.g. amounts of molecules), such as polynucleotides (e.g., DNA) or proteins (e.g., antibodies), by measuring the signal of a detectable reporter associated with hybridized polynucleotides or antigen / antibody complex. In the microfabricated device according to the invention, detection of the presence of molecules (i.e., polynucleotides, proteins, or antigen / antibody complexes) are correlated to a hybridization signal from an optically-detectable (e.g. fluorescent) reporter associated with the bound molecules. These hybridization signals can be detected by any suitable means, for example optical, and can be stored for example in a computer as a representation of the presence of a particular gene. Hybridization probes can be immobilized on a substrate that forms part of or is exposed to a channel or channels of the device that form a closed loop, for circulation of sample to actively contact complementary probes. Universal chips according to the invention can be fabricated not only with DNA but also with other molecules such as RNA, proteins, peptide nucleic acid (PNA) and polyamide molecules.

Surgical instruments for treating gastro-esophageal reflux

Instruments for thermally-mediated treatment of a patient's lower esophageal sphincter (LES) to induce an injury healing response to thereby populate the extracellular compartment of walls of the LES with collagen matrices to altere the biomechanics of the LES to provide an increased intra-esophageal pressure for preventing acid reflux. A preferred embodiment is a bougie-type device for trans-esophageal introduction that carries conductive electrodes for delivering Rf energy to walls of the LES (i) to induce the injury healing response or (ii) to "model" collagenous tissues of the LES by shrinking collagen fibers therein. Typically, an Rf source is connected to at least one conductive electrode that may be operated in a mono-polar or bi-polar fashion. A sensor array of individual sensors is provided in the working end. A computer controller is provided, which together with feedback circuitry, is capable of full process monitoring and control of: (i) power delivery; (ii) parameters of a selected therapeutic cycle, (iii) mono-polar or bi-polar energy delivery, and (iv) multiplexing of current flow between various paired electrodes. The controller can determine when the treatment is completed based on time, temperature, tissue impedance or any combination thereof.

Vision catheter

A catheter with a small optical fiber or bundle of fibers includes a scanning mechanism constructed with the use of any vibration capable component. Magnetic, piezoelectric or other mechanisms are used to vibrate the end of the fiber and thus create a scanning effect which extends the field of view. One or more lenses may be utilized, including a lens attached to the distal tip of the image fiber, or a lens attached to the distal tip of the catheter for creating an image plane which can be scanned by the fiber. In one embodiment, multiple light sources may be connected to the fiber for enabling the use of field sequential color techniques for real-time imaging, as well as real-time fluorescent imaging for disease detection. A photodetector assembly connected to the proximal end may contain both filtered and unfiltered detectors for use with both standard imaging and fluorescent imaging. The resulting vision catheter is relatively inexpensive and disposable.

A CCD, CMOS, or photodiode camera at the proximal end of the fiber captures the image and transfers it to a computer or processor. A light splitter coupled to a light source provides light through an illumination fiber. The resulting vision catheter is relatively inexpensive and disposable.
