An improved system and method for macroscopic and microscopic surgical navigation and visualization are presented. In exemplary embodiments of the present invention an integrated system can include a computer which has stored three dimensional representations of a patient's internal anatomy, a display, a probe and an operation microscope. In exemplary embodiments of the present invention reference markers can be attached to the probe and the microscope, and the system can also include a tracking system which can track the 3D position and orientation of each of the probe and microscope. In exemplary embodiments of the present invention a system can include means for detecting changes in the imaging parameters of the microscope, such as, for example, magnification and focus, which occur as a result of user adjustment and operation of the microscope. The microscope can have, for example, a focal point position relative to the markers attached to the microscope and can, for example, be calibrated in the full range of microscope focus. In exemplary embodiments of the present invention, the position of the microscope can be obtained from the tracking data regarding the microscope and the focus can be obtained from, for example, a sensor integrated with the microscope. Additionally, a tip position of the probe can also be obtained from the tracking data of the reference markers on the probe, and means can be provided for registration of virtual representations of patient anatomical data with real images from one or more cameras on each of the probe and the microscope. In exemplary embodiments of the present invention visualization and navigation can be provided by each of the microscope and the probe, and when both are active the system can intelligently display a microscopic or a macroscopic (probe based) augmented image according to defined rules.

There is proposed an apparatus for eye surgery, which comprises a stand (24) having a stand base (32) that is movable or realized for mounting on a wall or ceiling, and having a stand arm arrangement (34, 36) that is manually adjustable, at least partially, relative to the stand base, an operation microscope (38) being attached to the stand arm arrangement. Further, the eye-surgery apparatus comprises a laser appliance, which provides pulsed, focused laser radiation having radiation properties suited to the application of incisions in the human eye (14). The laser appliance comprises a laser source (20) and a laser treatment head (26) that is attached to the stand arm arrangement (34, 36) and emits the laser radiation, a flexible transmission fiber (22) or a jointed beam transport arm being provided for the purpose of transporting the laser radiation to the laser treatment head. The laser treatment head (26) is positioned or positionable in an observation beam path of the operation microscope (38) and provides a passage (52) for an observation beam going along the observation beam path. The method comprises the steps of disinfecting a fresh animal eyeball; treating with filter paper dipped with alcohol after disinfecting, and then erasing an epithelial cell layer; incising the corneal limbus under an operation microscope, stretching into an iris restorer to separate the anterior cornea, and then shearing the anterior cornea with corneal scissors; drilling the lamellar cornea with a corneal annulus; soaking the fresh lamellar cornea into serum, and then rinsing or performing ice-bath electrophoresis treatment; performing gradient dehydration after the treatment to obtain a non-dried lamellar tissue engineering corneal frame, sterilizing with ethylene oxide, and preserving for later use; drying the non-dried cornea matrix sheet in a 24-pore plate to obtain dried decellularized lamellar cornea; and sterilizing the dried sample with cobalt 60, and performing rehydration treatment to obtain a rehydrated lamellar cornea matrix sheet. The obtained cornea matrix sheet is high in transparency, low in structural destroy, good in biocompatibility, close to fresh cornea in performance and thorough in decellularization.

The operation microscope includes an objective lens opposed to an eye to be operated, a front lens which is provided between the eye to be operated and an anterior focal point of the objective lens and condenses illumination light to illuminate an interior of the eye to be operated, and the observation prism disposed near the front lens to observe fundus and its surroundings of the eye to be operated. An oblique surface (refractive surface) of the observation prism has a curved shape in which the curved shape shows a certain negative curvature in a direction to an intersection with a refractive surface. Therefore, aberrations caused on images of entrance pupils of an observation system and an image of an exit pupil of an illumination system can be corrected. The images of the entrance pupils and the image of the exit pupil are sufficiently separated from one another on a cornea of the eye to be operated, so that an observation with less glare can be conducted.
