Polyaxial bone screw with shank articulation pressure insert and method

A polyaxial bone screw assembly includes a threaded shank body having an upper portion, a receiver member or head, a retaining and articulating structure, and a pressure insert disposed between the shank upper portion and a rod. The receiver has a U-shaped cradle defining a channel for receiving a spinal fixation rod. The channel communicates with a receiver cavity and further with a restrictive opening that allows for loading the shank upper portion into the receiver but prevents passage of the retaining and articulating structure out of the receiver. The retaining and articulating structure has an external substantially spherical surface that mates with an internal surface of the receiver cavity, providing a ball joint, enabling the receiver to be disposed at an angle relative to the shank body. The pressure insert presses upon the shank upper portion and not the retaining and articulating structure and is either side or top loadable. The insert engages with an inner surface of the receiver to provide for the setting of an angle of articulation between the shank and the receiver. In a ratcheted embodiment, the insert may be used to independently lock the angle of orientation between the shank and the receiver.

Helical guide and advancement flange with radially loaded lip

A spinal fixation device combines an open-headed anchor member, such as a bone screw or a hook, with a closure member to thereby clamp a spinal fixation rod to the anchor member. The anchor member has spaced apart arms forming a rod receiving channel. The arms have arm extensions or tabs connected to main portions of the arms by weakened regions to enable the tabs to be broken-off or separated after the rod is clamped. The closure member and inner surfaces of the arms and tabs have helical anti-splay guide and advancement interlocking flanges formed thereon which cooperate to prevent splaying the arms and extensions as the closure member is advanced into the rod receiving channel. The flanges have anti-splay contours which can be formed on load flanks or stab flanks of the flanges. The load flanks can be oriented in such a manner as to aid in the anti-splay characteristics of the flanges or to control the proportioning of axial stresses between the flanges.

Process for anchoring connecting elements in a material with pores or cavities and connecting elements therefor

A joining pin (3.2) with which two parts (1 and 2) made from a porous material, particularly wood or a wood-like material, are to be joined together, is anchored in the porous material at predetermined anchoring points (31, 33). For this purpose, a bore (4.2) with a closed inner end (41) is made in the parts (1 and 2). The shape of this bore (4.2) is so matched to the joining pin (3.2) that it can be introduced substantially without force expenditure into the bore and is positionable in a first position. At least one predetermined anchoring point (31, 33) between the joining pin (3.2) and the wall of the bore (4.2) is formed when pressure is built up by pressing the joining pin (3.2) with a pressing force (F) more deeply into the bore to a second position. Energy is supplied in a planned manner to the joining pin (3.2) so that at the predetermined anchoring points (31, 33) the thermoplastic material of the joining pin (3.2) is plasticized. The locally plasticized plastic material is pressed by the local pressure into the porous material of the parts and forms local, macroscopic anchors (10, 20). The joining pin (3.2) is, e.g., made entirely from a thermoplastic material and the energy for plasticizing is supplied thereto by ultrasonic vibration.
