Microporation of tissue for delivery of bioactive agents

A method of enhancing the permeability of a biological membrane, including the skin or mucosa of an animal or the outer layer of a plant to a permeant is described utilizing microporation of selected depth and optionally one or more of sonic, electromagnetic, mechanical and thermal energy and a chemical enhancer. Microporation is accomplished to form a micropore of selected depth in the biological membrane and the porated site is contacted with the permeant. Additional permeation enhancement measures may be applied to the site to enhance both the flux rate of the permeant into the organism through the micropores as well as into targeted tissues within the organism.

Method of applying acoustic energy effective to alter transport or cell viability

A method for reversibly, or irreversibly, altering the permeability of cells, tissues or other biological barriers, to molecules to be transported into or through these materials, through the application of acoustic energy, is enhanced by applying the ultrasound in combination with devices for monitoring and / or implementing feedback controls. The acoustic energy is applied directly or indirectly to the cells or tissue whose permeability is to be altered, at a frequency and intensity appropriate to alter the permeability to achieve the desired effect, such as the transport of endogenous or exogenous molecules and / or fluid, for drug delivery, measurement of analyte, removal of fluid, alteration of cell or tissue viability or alteration of structure of materials such as kidney or gall bladder stones. In the preferred embodiment, the method includes measuring the strength of the acoustic field applied to the cell or tissue at the applied frequency or other frequencies, and using the acoustic measurement to modify continued or subsequent application of acoustic energy to the cell or tissue. In another preferred embodiment, the method further includes simultaneously, previously, or subsequently exposing the cell or tissue to the chemical or biological agent to be transported into or across the cell or tissue. In another preferred application, the method includes removing biological fluid or molecules from the cells or tissue simultaneously, previously or subsequently to the application of acoustic energy and, optionally, assaying the biological fluid or molecules.

Applications of HIFU and chemotherapy

A method, using high intensity ultrasound, which may also be combined with a chemotherapy agent, that can result the direct destruction of tumor cells and in the reduction or elimination of local reoccurrence of cancer after removal of cancerous tissue, such as a surgical breast lumpectomy or surgical excision of a brain tumor. The method comprises either (1) the treatment of the tumor directly with High Intensity Focused Ultrasound, or (2) the treatment of the margins of the tissue surrounding the surgical cavity or void with ablative continuous wave high intensity ultrasound and a combination of a locally delivered chemotherapy agent and high intensity ultrasound, termed sonoporation. The present invention permits both the direct destruction (ablation) of tumor tissue as well as the destruction of tissue around the surgical margin and may include the enhanced local cellular uptake of locally injected chemotherapeutic drugs, all of which can be accomplished by a therapeutic ultrasound device used during surgery.

Using Alternating Electric Fields to Increase Cell Membrane Permeability

Certain substances (e.g., large molecules) that ordinarily cannot traverse the cell membrane of cells can be introduced into cells by applying an alternating electric field to the cell for a period of time, wherein the frequency of the alternating electric field is selected so that application of the alternating electric field increases permeability of the cell membrane. Once the permeability of the cell membrane has been increased, the substance is able to cross the cell membrane. This approach is particularly useful in the context of cancer cells (e.g., glioblastoma).
