Localized production of microbubbles and control of cavitational and heating effects by use of enhanced ultrasound

The invention is a method of using ultrasound waves that are focused at a specific location in a medium to cause localized production of bubbles at that location and to control the production, and the cavitational and heating effects that take place there. According to the method of the invention, the production and control is accomplished by selecting the range of parameters of multiple transducers that are focused at the location such that they produce by interference specific waveforms at the focal point, which are not produced at other locations. Preferably the region within the focal zone of all the transducers in which the specific waveform develops at significant intensities, is typically very small. The invention is also a system for carrying out the method. The method and system of the invention can be used to perform a variety of therapeutic procedures. Typical of such procedures is occlusion of varicose veins.

Ultrasound target vessel occlusion using microbubbles

Selective occlusion of a blood vessel is achieved by selectively damaging endothelial cells at a target location in the blood vessel, resulting in the formation of a fibrin clot proximate to the damaged endothelial cells. Additional fibrinogen can then be introduced into the blood vessel if occlusion is not achieved, as the fibrinogen is converted to fibrin by enzymes released by the exposed thrombogenic tissue and activated platelets. Endothelial cells are selectively damaged using thermal effects induced by ultrasound, by mechanical effects induced by ultrasound, or by mechanical effects produced by a tool introduced into the blood vessel (such as a catheter-based tool). A particularly preferred technique for selectively damaging endothelial cells involves introducing an ultrasound activatable agent into the blood vessel, and causing cavitation in that agent using pulses of high-intensity focused ultrasound having a duration insufficient to induce thermal damage in adjacent perivascular tissue.

The ablation manifold has a power transducer and a coupled leaky acoustic cavity. The leaky acoustic cavity contains a first portion of ultrasonic power emission for intra-cavity ablation while allowing a second portion of ultrasonic power emission to leak outside for an extra-cavity ablation. The power transducer and the leaky acoustic cavity are configured to form a confocal resonant cavity. An intervening bird cage is coupled to the resonant cavity for stronger resonance while maintaining ultrasound leakage. A protective shield is mounted around the waist of bird cage to strengthen resonance and to prevent damaging the intima from an otherwise normal incidence of high intensity ultrasound. A microbubble releasing device and micro pump are also included to further increase the ablation efficacy.

Disclosed herein are agents for enhancing the contrast in an ultrasound image. These agents are extremely small bubbles, or “microbubbles,” comprised of specially selected gases. The microbubbles described herein exhibit long life spans in solution and may be produced at a size small enough to traverse the lungs, thus enabling improved ultrasound imaging of the cardiovascular system and other vital organs. Also disclosed herein is a method for selecting gases from which contrast agents may be produced. The method is based on calculations using inherent physical properties of gases and describes a means to associate the properties of a gas with the time for dissolution of microbubbles comprised of the gas.
