Graft fixation system and method

A graft fixation device, system and method are disclosed for reconstruction or replacement of a ligament or tendon preferably wherein a soft tissue graft or a bone-tendon-bone graft is received and implanted in a bone tunnel. The graft fixation system includes a fixation device comprising a threaded body which is rotatably connected to a graft interface member. One embodiment of the implant/graft interface member includes an enclosed loop for holding a soft tissue graft. Another embodiment of the interface member includes a bone cage comprising a cage bottom and removable cage top to hold a bone block at one end of a bone-tendon-bone (BTB) graft. An additional embodiment of the interface member includes a one-piece bone cage which may be crimped or stapled to a bone block. The fixation device holds a graft in centered axial alignment in a bone tunnel. The body portion of the fixation device may be turned without imparting substantial twist to a graft attached to the device, due to the rotatable coupling between the threaded body and the interface member. The fixation device may be installed using a driver tool that has a shaft and an outer sleeve, wherein the driver may be used to twist the fixation device and independently exert a pushing or pulling force thereto. The graft fixation method may be used to install a fixation device by pulling or pushing it into a prepared bone tunnel while minimizing the possibility of abrasion or other damage to a graft attached to the fixation device.

Method and apparatus for use of a self-tapping resorbable screw

A bone attachment apparatus and implantation system. The attachment device provides a screw with channels formed therein for implantation within a bone aperture. The channels are used as a torque transfer surface during implantation, and cooperate with a thread forming tap to enable screw implantation simultaneously with thread formation within the aperture. The tap can be used to form channels within the bone. A staple is coupled to the screw and the bone, utilizing the respective channels formed therein, to prevent rotation therebetween. The screw can also cooperate with the staple to secure a soft tissue graft.

In one aspect, the graft collar comprises more than one region, wherein the regions can comprise different materials configured to promote integration of and the regeneration of the interfacial region between tendon and bone.This application also provides for scaffold apparatuses and methods for fixing musculoskeletal soft tissue to bone. The scaffold apparatus is multiphasic, preferably triphasic, and each phase is configured promote growth and proliferation of a different cell and its associated tissue. In one aspect, the scaffold apparatus is triphasic, with phases comprising materials to promote growth and proliferation of fibroblasts, chondroblasts, and osteoblasts. In addition, an apparatus comprising two portions, each of said portion being the scaffold apparatus described above is provided, wherein each of said portion encases one end of a soft tissue graft. Further, a triphasic interference screw is provided.This application further provides apparatuses and methods for inducing formation of fibrocartilage comprising wrapping a graft collar with polymer-fiber mesh configured to apply compression to the graft collar. A stent tissue graft prosthesis (20) for repairing, excluding and / or reinforcing a vessel, duct and the like in a patient. The prosthesis further includes an outer tubular member (25) such as an outer expandable stent (44) for retaining the tissue graft on the inner stent. The ends of the inner and outer stents are coincident with or extend beyond the ends of the tissue graft to prevent eversion or fold back of the tissue graft during withdrawal of a delivery catheter in a placement procedure.

The present invention provides a plasticized dehydrated or freeze-dried bone and / or soft tissue product that does not require special conditions of storage, for example refrigeration or freezing, exhibits materials properties that approximate those properties present in normal hydrated tissue, is not brittle, does not necessitate rehydration prior to clinical implantation and is not a potential source for disease transmission. The invention replaces water in the molecular structure of the bone or soft tissue matrix with one or more plasticizers allowing for dehydration of the tissue, yet not resulting in an increase in brittleness of the plasticized product, and resulting in compressive and / or tensile properties similar to those of normal hydrated bone. Replacement of the chemical plasticizers by water prior to implantation is not required and thus, the dehydrated bone or soft tissue plasticized product can be placed directly into an implant site without significant preparation in the operating room.
