Method for treating obstructive sleep disorder includes removing tissue from the base of tongue

InactiveUS7090672B2Stiffening the surrounding tissue structureUseful in treatmentSuture equipmentsHeart valvesThermal energyTongue root
A method for treating obstructive sleep disorders includes accessing the interior of the tongue through an incision made in the skin in the vicinity of the jaw of a patient; advancing an instrument through the incision into the interior of the tongue; and removing an amount of tissue from the interior of the base of the tongue with the instrument. The present invention includes forming a cavity or plurality of channels in the tongue. The cavity may be collapsed using a suture, fastener or bioadhesive. An emplaced suture may be provided to hold the cavity in a collapsed position thereby reducing the degree of obstruction. Additionally, thermal energy may be applied to the tissue surface immediately surrounding the channels to cause thermal damage to the tissue surface, thereby creating hemostasis and stiffening the surrounding tissue structure.

Method and apparatus for the non-invasive detection of particular sleep-state conditions by monitoring the peripheral vascular system

Method and apparatus for monitoring the sleep state condition of an individual by using an external probe applied to a peripheral body location, such as the individual's finger or toe, for detecting changes in the peripheral vascular bed volume of the individual. A predetermined pressure field is applied to the distal end of the peripheral body location, including its distal-most extremity, to prevent the occurrence of venous pooling within and distal to the peripheral body location. The probe produces an output corresponding to changes in the peripheral arterial bed volume at the peripheral body location, which output provides an indication of the sleep state condition of the individual. Such information is useful in diagnosing and / or in treating, a number of sleep disorders as well as other conditions, such as impotence, diabetes, and various disorders in children.

Systems and methods for electrosurigical treatment of obstructive sleep disorders

InactiveUS7004941B2Increase the apertureEffectively treating sleep apneaSurgical instruments for heatingTherapeutic coolingRadix linguaeMedicine
Systems, apparatus, and methods for selectively applying electrical energy to a target location within the head or neck of a patient for treating obstructive sleep disorders. A method of the present invention involves positioning an electrosurgical probe with respect to a target tissue that affects the aperture of the upper airway of the patient. For example, the position of the tongue and the radix linguae affect the upper airway. The position of the tongue is controlled by the genioglossus muscle and tendon. The tendon of the genioglossus muscle may be irreversibly shrunk by positioning an electrosurgical probe in at least close proximity to the tendon, and applying a suitable high frequency voltage to the probe in a sub-ablation mode. Controlled heating of the tendon is effected by application of the high frequency voltage to the probe, wherein the voltage is insufficient to ablate the tissue of the tendon. The controlled heating of the tendon effects contraction of collagen fibers of the tendon, whereby the tongue is advanced and / or depressed, and the aperture of the upper airway is increased.

A medical sleep disorder arrangement integrates into current diagnosis and treatment procedures to enable a health care professional to diagnose and treat a plurality of subjects suffering from insomnia. The arrangement may include both environmental sensors and body-worn sensors that measure the environmental conditions and the condition of the individual patient. The data may be collected and processed to measure clinically relevant attributes of sleep quality automatically. These automatically determined measures, along with the original sensor data, may be aggregated and shared remotely with the health care professional. A communication apparatus enables the healthcare professional to remotely communicate with and further assess the patient and subsequently administer the treatment. Thus, a more accurate diagnosis and more effective treatment is provided while reducing the required clinician time per patient for treatment delivery.

Airway monitoring apparatus 30 includes a driver 6 for applying sound in the audible frequency range to the airway of a subject and a detector 7 for monitoring the response of the airway, e.g. by detecting transmitted sound signal components and / or reflected sound signal components. Variations in the detected sound signal, e.g. energy, due to attenuation of the signal, give an indication of the state of the airway, e.g. airway patency, and can be used to monitor sleep-disordered breathing events, such as apnea and hypopnea, and to provide a breathing event index, e.g. AHI. The apparatus may provide servo-control for a respiratory assist device, such as a positive airway pressure device, so as to provide an appropriate pressure level setting. It may allow for home use of such devices and for home titration. It may also assist in discriminating central and obstructive breathing events, and in providing a measure of airway resistance.
