A vascular interventional device may be introduced over a guidewire into a vessel of the cardiovascular system of a patient. This device includes a hollow, flexible tube having a proximal end and a distal end that is adapted to selectively engage a target vessel of the cardiovascular system of the patient. This tube also includes a lumen that is continuous from the proximal to the distal end, and has an end hole in the distal end that is in fluid communication with the lumen. A plurality of side holes are provided near the distal end of the tube, each of which is in continuous fluid communication with the lumen. The device also includes a hollow vessel dilator that is adapted for insertion into and through the tube and over the guidewire. The dilator has an inside diameter that is slightly larger than the guidewire and an outside diameter that is slightly smaller than the diameter of the lumen of the tube. The distal end of the dilator is adapted to accommodate vascular entry over the guidewire, and the dilator is adapted to dilate the vessel to accept the tube. The device also includes a hub at the proximal end of the tube. The hub includes an end port through which a second interventional device having an outside diameter smaller than the diameter of the lumen may be introduced into the lumen of the tube. The hub also includes a side port through which a fluid agent may be injected for delivery through the lumen and out the end hole and side holes of the tube. A sealing mechanism is also provided in the hub to prevent air from entering the tube and blood and other fluids from leaking out of the tube through the hub. A pair of vascular interventional devices, at least one of which is constructed according to the invention, may be utilized to treat or study a cardiovascular condition, or to measure the blood pressure across a vascular segment.

A vascular interventional device may be introduced over a guidewire into a vessel of the cardiovascular system of a patient. This device includes a hollow, flexible tube having a proximal end and a distal end that is adapted to selectively engage a target vessel of the cardiovascular system of the patient. Telescoping vascular dilator

A telescoping dilator assembly is configured for percutaneous insertion of large diameter intravascular devices into a blood vessel while avoiding kinking of a guidewire and minimizing blood loss. The assembly includes a first smaller diameter dilator with an inner lumen that fits snugly over a guidewire. The smaller dilator may be tapered on both ends. A second larger diameter dilator is configured to slide over the first smaller dilator. A tear away sheath is configured to slide over the dilators and into a blood vessel. A tear away sheath plug is configured to form a seal at a proximal end of the sheath. When the dilators are removed from the blood vessel, an intravascular device may be passed through the sheath into the blood vessel.

The present invention relates to a dilator. The dilator includes a shaft, a lumen and a hub. The shaft has at least two stiffness sections with the stiffness sections becoming less rigid as they approach a distal end of the shaft. The varying stiffness sections allow the dilator to track along the guidewire through torturous vasculature so as the dilator is advanced and the stiffness is changed, the stiffer sheath can advance smoothly through a blood vessel. The dilator may also taper at the distal tip to stretch the initial skin puncture hole larger to accommodate the dilator shaft and ease the sheath tip insertion through to the vasculature. Or the shaft may taper from where it extends beyond the sheath distal tip to the distal end so that it can reach further distal in the vasculature. The dilator may also include an atraumatic tip for easier advancement of the dilator. The dilator distal end may also include one or more radiopaque markers and a shaped distal end.

A vessel access and closure device places a running suture in the wall of a blood vessel using a rotating helical suture needle that carries the suture along a helical path passing through the vessel wall, then reverses rotation to release a suture anchor attached to the distal end of the suture. A loose helical coil of suture is left behind as the helical suture needle withdraws. The device is withdrawn and replaced with a vessel dilator and an introducer sheath that opens up a larger access opening into the blood vessel and creates a pathway that passes though the helical coil of suture for introducing an interventional device into the blood vessel. After the interventional procedure is completed, the interventional device and the introducer sheath are withdrawn and the running suture is tightened and secured with a suture lock to close the access opening into the vessel.

The biasing element is adapted and configured for urging the dilator in the retracted position and the retaining pin is position so as to secure the dilator in the extended position.

A single needle vascular access device includes a body having an upper and lower passage with both in fluid communication with a vascular dilator.
