Suturing Device, System, and Method

Improved medical suturing devices, systems, and methods may hold a suture needle at a fixed location relative to a handle of the device, allowing the surgeon to grasp and manipulate the handle of the suturing device to insert the needle through tissues in a manner analogous to use of a standard needle gripper. Cycling the handle from a closed position to an open position and back to the closed position may alternate the device between gripping the needle with a first clamp (for example, along a proximal portion of the needle) to gripping the needle with a second clamp (for example, along a distal portion of the needle) and optionally back to gripping with the first clamp, with the needle often staying at a substantially fixed location relative to the suturing device body. Related single-clamp needle grasping devices can be bent plastically by a surgeon, and / or have bodies that are grasped by a hand while a portion of the hand actuates a handle.

Controlled steering of a flexible needle

A robotic system for steering a flexible needle during insertion into soft-tissue using imaging to determine the needle position. The control system calculates a needle tip trajectory that hits the desired target while avoiding potentially dangerous obstacles en route. Using an inverse kinematics algorithm, the maneuvers required of the needle base to cause the tip to follow this trajectory are calculated, such that the robot can perform controlled needle insertion. The insertion of a flexible needle into a deformable tissue is modeled as a linear beam supported by virtual springs, where the stiffness coefficients of the springs varies along the needle. The forward and inverse kinematics of the needle are solved analytically, enabling both path planning and correction in real-time. The needle shape is detected by image processing performed on fluoroscopic images. The stiffness properties of the tissue are calculated from the measured shape of the needle.

Safety needle assembly with correct medication connection

To ensure that there is no misconnection between a needle, such as for example a spinal or epidural needle, and the appropriate medication, the needle hub the needle is formed, by molding or extrusion, to have a given configuration. An adapter is placed between the needle and a conventional fluid store, or medical line. The end of the adapter that connects to the needle assembly is formed to have a configuration that is complementary to that of the hub of the needle assembly, so that the adapter and the needle hub may be readily mated to each other. The other end of the adapter is formed to have another configuration, for example a conventional luer, that is readily connectable to a conventional fluid store, such as a syringe that has a conventional luer. The adapter is formed by inter-fitting two elements to effect an integral locking mechanism to prevent the uncoupling of the fluid store and the adapter, once they are securely coupled. By thus configuring the hub of the needle to have a given configuration that is matable only to the end of an adapter that has a complementary configuration, and further providing an adapter that prevents the removal of the fluid store connected thereto, the potential for mis-connecting the needle to a fluid store that contains a different medicament is greatly reduced, if not totally eliminated.

Methods and systems for spinal radio frequency neurotomy

Methods and systems for spinal radio frequency neurotomy. Systems include needles capable of applying RF energy to target volumes within a patient. Such target volumes may contain target medial branch nerves along vertebrae or rami proximate the sacrum. Such procedures may be used to ablate or cauterize a portion of the targeted nerve, thus blocking the ability of the nerve to transmit signals to the central nervous system. Disclosed needles may be operable to asymmetrically, relative to a central longitudinal axis of the needle, apply RF energy. Such asymmetry facilitates procedures where a tip of the needle is placed proximate to anatomical structures for location verification. Then RF energy may be applied in a selectable direction relative to the needle tip to ablate volumes that include the targeted medial branch nerves or rami, thus denervating facet joints or the sacroiliac joint, respectively, to relieve pain in a patient.
