An identification tag and related tag system are provided for securely mounting the identification tag onto a selected wearer or object, such as mounting onto a wearer's wrist or the like. The identification tag is adapted to receive wearer-related identification and other information in human readable and/or machine readable form, and for slide-fit assembly onto an elongated flexible strap such as a wristband or bracelet for mounting onto the selected wearer or object. In one preferred form, the identification tag includes a radio frequency identification (RFID) circuit adapted for communicating wearer-related information with a remote reader. The tag system, including the identification tag assembled with the flexible strap, is particularly useful with a small wearer or object, such as an infant, to provided a relatively extensive and relatively flat information-bearing surface area.

An insertion device and insertion set. The insertion device for inserting at least a portion of at least one piercing member of an insertion set through the skin of a patient includes a device housing, a carrier body and a driver. The carrier body is slidably received within the device housing for movement between an advanced position and a retracted position. The carrier body also includes a receiving structure to support the insertion set in a position with the at least one piercing member oriented for insertion through the skin of the patient at a predetermined or variable angle relative to the skin of the patient upon movement of the carrier body from the retracted position to the advanced position. An improved LED assembly comprises a frame and a circuit substrate. The frame has a slot, which has an opening on the upper surface of the frame. An elongated inverted L-shaped structure is formed on either side of the opening, and one or more conductive strips are formed on the inner side of the elongated inverted L-shaped structure.
