Rapid exchange iol insertion apparatus and methods of using

A system for easily transferring an intraocular lens (IOL) from a lens case to an inserter, and then into a patient's eye. The lens case has a transfer mechanism therein which retains the IOL until engagement with the inserter. The transfer mechanism may include jaws having a closed configuration for retaining the IOL and an open configuration for releasing the IOL. Engagement of the inserter with the lens case automatically opens the jaws and transfers the IOL to the inserter. The IOL is transferred into a load chamber of a nosepiece rotatably coupled to a handpiece. After transfer of the IOL, the nosepiece is rotated from a load position to a delivery position. The IOL may have an optic and a haptic coupled to the optic, and the lens case may be capable of configuring the haptic as desired to facilitate its transfer into an inserter and / or into the eye. For instance, the lens case may fold one or both of the haptics over the optic. Preferably, the lens case maintains the haptic in this position during transfer of the intraocular lens into an inserter and / or inserter cartridge. A manifold for easily distributing a viscoelastic medium to the load chamber of the inserter is also provided.

Intraocular lens inserter system components

Disclosed are an intraocular lens (IOL) push rod, an IOL cartridge, multiple embodiments of an IOL cartridge housing, and a method for folding an IOL for insertion into an eye during ocular surgery. The distal end of the push rod is contoured to apply force to a substantial portion of the perimeter of an IOL to advance the IOL through a bore of the cartridge. Two hinges couple flanges on the IOL cartridge to a central portion that supports an IOL when protected by a cover for an extended storage time. The IOL cartridge has a locking element that engages a cartridge housing or IOL inserter. Each cartridge housing accepts the IOL cartridge with the IOL in an unfolded state. The cartridge bore is unobstructed and is tapered to fold one side of the IOL over the other as the IOL is advanced through the bore and into an eye.

Intraocular lens inserter

An intraocular lens (IOL) inserter and method of use ensure that an artificial intraocular lens is controllably ejected from a cartridge and into a patient's eye by using successive, predefined detent positions. One embodiment has a distal pivotal portion that can be pivoted to a desired angle as a mover advances the IOL into the eye. A detent controls the motion of the mover. In one embodiment, the detent includes a longitudinally extending component with a plurality of stair steps, so that a face of each stair step engages the housing at each different predefined detent position. Other embodiments include at least one orifice in the housing, so that when the orifice is aligned with a pin or protrusion, it engages the orifice to define a detent step. Another embodiment includes a spring-loaded collar that biases a pin into a predefined detent step.

Intraocular lens insertion plunger with low stimulus soft tip

An intraocular lens (IOL) insertion apparatus for implanting IOLs through smaller incisions. The insertion apparatus includes an insertion cartridge that receives the IOL and cooperates with a handpiece. The cartridge includes a longitudinal lumen from a loading chamber to an open distal mouth that gradually narrows in dimension. A push rod having an extremely soft tip thereon urges the IOL through the cartridge and from the open distal mouth. The soft tip is made of low-stimulus material having a minimum ultimate elongation of 400%. Because of the extremely soft material of the tip, the outside diameter of the open distal mouth can be reduced to no more than 2.0 mm, enabling passage through incisions of 2.2 mm or less. The soft tip may be a thermoplastic elastomer having a relatively high elongation and relatively low modulus at elongations of 100-300%.

Multi-action device for inserting an intraocular lens into an eye

A device for inserting an intraocular lens (IOL) into an eye includes a tubular body member with an inner surface and a plunger with an gripping device that is receivable within the body member. The gripping device includes a locking member and is configured such that when urged longitudinally in a distal direction, the gripping device is movable in the distal direction within the body member. However, when urged longitudinally in a proximal direction, the gripping device is prevented from moving in the proximal direction by the locking member engaging with the inner surface of the body member. As such, inadvertent movement of the plunger in the proximal direction caused by pulling back on the plunger is substantially prevented. If proximal movement of the plunger is desired, then the plunger may include handle that is operatively coupled to the gripping device such rotation of the handle causes the handle to move longitudinally in the body member. Accordingly, the plunger may be advanced or moved distally by longitudinally pushing the handle and / or by rotating the handle.

Methods and apparatus for inserting an intraocular lens into an eye

Apparatus and methodology for inserting an intraocular lens (IOL) into an eye. The apparatus includes a cartridge for holding an IOL and a handpiece with a body member for receiving the cartridge, an injector rod that is receivable by the body member, and a drive mechanism that longitudinally oscillates the injector rod in the body member when a rotational force is applied about a longitudinal axis in a single direction. In other words, a constant rotation in a single direction of the drive mechanism causes the injector rod to move distally and proximally in discrete increments, with the distal increments being larger than the proximal increments. Accordingly, the injector rod urges an IOL out of the handpiece in a two-steps-forward, one-step-back manner, thereby minimizing or eliminating the chance of the injector rod engaging with the IOL.
