Described are implantable devices and methods for treating various disorders of the pelvic floor by means of electrical stimulation of the pudendal or other nerves, and optional means for delivering drugs in association therewith. A method of precisely positioning and implanting a medical electrical lead so as to provide optimal stimulation of the pudendal nerve or a portion thereof is also described. Placement of a stimulation lead next to or on the pudendal nerve may be performed using conventional prior art techniques through gross anatomical positioning, but usually does not result in truly optimal lead placement. One method of the present invention utilizes neurophysiological monitoring to assess the evoked responses of the pudendal nerve, and thereby provide a method for determining the optimal stimulation site. Additionally, one or more electrical stimulation signals are applied, and optionally one or more drugs are infused, injected or otherwise administered, to appropriate portions of a patient's pelvic floor and pudendal nerve or portions thereof in an amount and manner effective to treat a number of disorders, including, but not limited to, urinary and / or fecal voiding dysfunctions such as constipation, incontinence disorders such as urge frequency and urinary retention disorders, sexual dysfunctions such as orgasmic and erectile dysfunction, pelvic pain, prostatitis, prostatalgia and prostatodynia.

Tension free pelvic floor repair

A kit and method for supporting a pelvic floor of a person with stress urinary incontinence or other symptoms. The method involves using barbed sutures as part of a sling for supporting the pelvic floor or urethra of a person experiencing stress urinary incontinence or other symptoms. A surgeon implants the sling along with the barbed sutures, using one of several known methods. The barbed sutures may be interwoven with the sling and the supports for the sling, so that once the sling and its supports are placed into the patient, barbs of the sutures will resist movement. The sutures may also be woven into the supports to resist movement.

The device comprises a probe having a pressure sensor and a vibrator, and a microprocessor connected with the sensor and the vibrator. Initially, the probe is inserted into a person's vagina or rectum and the highest force that can be applied by the person contracting the pelvic muscles is ascertained and registered. The contraction of the pelvic floor muscles is repeated at intervals, and the force applied by pelvic floor muscles at each contraction is ascertained and compared with the registered highest value. The vibrator is activated at each contraction of the floor muscles only if the force has a predetermined relationship to the registered value, and only for a predetermined period as long as the predetermined relationship is maintained.

Multi-mode pelvic exercise probe

A perineometer exercise probe for home or clinical use assesses the strength of pelvic floor muscles and provides audible, visual and tactile biofeedback signals as training aids during pelvic exercises. The exercise probe is selectively operable in a passive reaction mode, in which audible, visual and tactile biofeedback signals proportional to pelvic muscle contractions are generated, and in an active vibrating mode in which therapeutic vibrations are applied directly to internal pelvic musculature with or without co-generation of biofeedback signals proportional to the strength of pelvic muscle contractions; and in a combination of both modes simultaneously. The probe reacts the pelvic contraction forces and thus provides passive tactile feedback signals that are experienced simultaneously with audible, visual and vibration-induced tactile biofeedback signals for improving the endurance and strength of pelvic floor muscles. Biofeedback signaling is facilitated by wireless two-way communication between the probe and a portable monitor.

Biofeedback apparatus using magnetic stimulator and control method therefor

Provided is a biofeedback apparatus using a magnetic stimulator. In the biofeedback apparatus, a tube filled with a non-conductive fluid is disposed between a magnetic stimulator and patient's pelvic floor muscles to measure a change in pressure according to muscle exercise of the pelvic floor muscles, biofeedback is available without insertion of a tool for measurement of a pressure or an EMG (electromyogram) into vagina, urethra, or the like. In the biofeedback apparatus, since driving of the magnetic stimulator and driving of a pressure transducer do not influence each other, a controller can continuously monitor a state of change in pressure of the pelvic floor muscles by using the pressure transducer and, at the same time, can adjust a strength of a magnetic field generated from the magnetic stimulator to an optimal strength according to the monitored state, so that it is possible to maximize the effect of treatment.
