A surgical staple-clip including a clip component and a securing member is used in a wide range of surgical procedures. The staple-clip may be introduced to a surgical site in an un-assembled condition through a small port or trocar. An applier for the staple-clip comprising a pair of opposed jaw-like channels is provided to position and apply the clip component and the securing member. The clip component is positioned around a target tissue and is compressed or clamped upon the tissue using only the force required for a specific surgical procedure such as occlusion, ligation or fixation. When the clip component is properly applied, the securing member is urged forward and over the clip component to secure the staple-clip. The clip component may include traction enhancement features such as surface interruptions, bumps, valleys and ridges. With the staple-clip of the invention, the force required to constrict or occlude the tissue is separate from the force required to secure and maintain the staple-clip in position and, as a result, the body tissue is not over-compressed and nourishment to the body tissue is maintained. Other aspects of the invention include thumb actuated clip appliers for use in hand assisted laparoscopy (HAL). In one embodiment, a clip applier includes a handle and a thumb actuated mechanism that is used to slidably release clips onto a body tissue or vessel by sliding the thumb actuated mechanism forward and backward using only one hand. In another aspect of the invention, a two-stage clip is disclosed having a clip component and a staple component for securing the clip after it has been properly positioned.

Surgical access device with pendent valve

A surgical access device, such as a trocar, includes a pendent valve having an elongate structure extending from a proximal end to a septum valve disposed at a distal end. In operation, the elongate structure follows the angle of the instrument to pre-position the septum valve into the path of the instrument where it is not significantly challenged during instrument insertion or manipulation. The pendant valve can be made to float at both the proximal end and the distal end of the elongate structure, to further reduce the vulnerability of the septum valve. Since the valve is less vulnerable to instrument insertion, it can be formed to minimize friction and maximize the functional range of the access device.

The invention provides methods and systems for introducing a delivery device in the heart at or near the apex of the heart, wherein the delivery device includes a prosthesis, advancing the prosthesis to the target site, and disengaging the prosthesis from the delivery device at the target site for implantation. Specifically, the present invention provides valve replacement systems for delivering a replacement heart valve to a target site in or near a heart. The valve replacement system comprises a trocar or other suitable device to penetrate the heart at or near the apex of the heart, a delivery member that is movably disposed within the trocar, and a replacement cardiac valve disposed on the delivery member. The delivery member may further comprise mechanical or inflatable expanding members to facilitate implantation of the prosthetic valve at the target site.

Surgical Stapling Device for Performing Circular Anastomoses

The present disclosure is directed to a surgical stapling device (10) for performing circular anastomoses. The surgical stapling device includes a handle portion (12), an elongated body portion (14) and a head portion (16) including an anvil assembly (30) and a shell assembly (31). The handle portion (12) includes a rotatable approximation knob (22) for approximating the anvil and shell assemblies (30,31) and a firing trigger (20) for actuating a firing mechanism for ejecting staples positioned within the shell assembly (31). The firing trigger (20) forms one link of a two bar linkage provided to actuate the firing mechanism. The anvil assembly (30) includes a tiltable anvil which will tilt automatically after firing of the device and unapproximating the anvil and shell assemblies (30,31). The head portion (16) also includes a retractable trocar assembly (240) which is slidably positioned within an anvil retainer (38) and is automatically advanced and retracted upon attachment and detachment of the anvil assembly (30) onto the anvil retainer (38). A lockout tube (270) is provided and is positioned about the anvil retainer (38) for releasably engaging an anvil assembly (30). The lockout tube (270) prevents inadvertent detachment of the anvil assembly (30) from the anvil retainer (38). The surgical stapling device (10) also includes a firing lockout assembly which prevents actuation of the firing trigger (20) until an anvil has been attached to the device and the anvil (129) has been approximated. A tactile indication mechanism is also provided for notifying a surgeon that the device has been fired and for notifying a surgeon that the anvil head has been unapproximated a distance sufficient to permit the anvil head to tilt.

Differential fluidic actuators or mechanical cam bars deflect a distal end (e.g., rack, cam point) of the bending member to effect articulation. Thereby, the end effector may act upon tissue that would otherwise be obscured, such as behind an organ. The articulated end effector also advantageously allows an endoscope to be positioned behind the end effector without being blocked by the instrument shaft.

An ultrasonic surgical apparatus which can be used in laparoscopic surgery. The ultrasonic surgical apparatus includes an inserting unit and an operation unit. The inserting unit is inserted into a guide member called a trocar. The inserting unit includes an elongated member, a tip portion, and also a joint section provided between the elongated member and the tip portion. The tip portion includes a transducer unit and an oscillation member, and preferably further includes a clamp member and an open / close mechanism. The joint section enables the tip portion to be slanted with respect to the elongated member. The operation unit includes a mechanism for operating the joint section, and preferably further includes a mechanism for operating the clamp member. The direction of the oscillation member can be varied by the joint section. Because ultrasonic oscillation is generated in the tip portion, effective transmission of the ultrasonic oscillation to the oscillation member can be achieved.

A trocar adapted to extend across the body wall into a body cavity, forming a seal around an instrument inserted through the trocar, comprises a cannula having a first axis extending between proximal and distal ends. A valve housing attached to the proximal end of the cannula forms a working channel with the cannula and includes a rigid housing portion and a flexible housing portion. First and second septum valves are formed in an end wall of the flexible housing portion and form separate valve sets with associated zero closure valves disposed within the valve housing. The valve sets are sized and configured accommodating different ranges of instrument diameters. The first and second septum valves are offset from the axis of the cannula in normal positions but are movable to positions more aligned with the axis of the cannula, due to elastomeric characteristics of the flexible housing.

Laparoscopic instruments and trocar systems and related surgical method

Laparoscopic instruments and trocars are provided for performing laparoscopic procedures entirely through the umbilicus. A generally C-shaped trocar provides increased work space between the hands of the surgeon as well as S-shaped laparoscopic instruments placed through the trocar when laparoscopic instrument-trocar units are placed through the umbilicus. In order to facilitate retraction of intra-abdominal structures during a laparoscopic procedure, an angulated needle and thread with either one-or two sharp ends is provided. Alternatively, an inflatable unit having at least one generally C-shaped trocar incorporated within the unit's walls can be placed through the umbilicus following a single incision. Generally S-shaped laparoscopic instruments may be placed through the generally C-shaped trocars to facilitate access to intra-abdominal structures.
