Problems solved by technology

[0004] 1. In the space of the vena cava - filter may be "plugged" laterally immediately after implantation or over time;
[0005] 2. Difficulty in removing the filter from the vena cava, because it becomes difficult and sometimes impossible to fit the ring of the extractor on the snap-fit device of the filter body;
[0006] 3. The filter capacity of the filter placed at an angle (side blockage) is reduced;
[0008] 5. Filter tows (especially fixed tows) extend beyond the range of the vena cava into the surrounding tissue over time, penetrating the vein wall, which may lead to phlebitis and vena cava thrombosis at the filter level;
[0009] 6. Known kits do not allow accurate measurement of blood vessel dimensions with any x-ray instrument, even with a program for calculating the linear dimensions of veins
[0012] 1. Possibility of inclination when installing the centering element that first protrudes from the conduit, which leads to a reduction in the filtering capacity of the filter
[0013] 2. The vein may bend when the catheter exits the centering element, which causes disruption of the laminar flow of blood and leads to thrombosis
[0015] The disadvantage of this known IV filter is that removal of the filter causes trauma when the sharp hooks holding the tow are disengaged from the vena cava wall

example 1

[0088] Example 1. Female patient М, 76 years old, suffered right iliofemoral vein thrombosis and thrombus floaters spreading to the right external and common iliofemoral veins with sequelae of thrombosis in the left lower extremity, as well as intermittent thrombosis of the pulmonary artery and developing pneumococcal infection. A left subclavian phlebotomy performed on the degenerative iliofemoral hologram revealed a prominent rod-shaped curvature of the inferior vena cava. The true diameter of the inferior vena cava measured with the accessories already in the kit was 22 mm. Immediately at the level of the depression of the inferior vena cava farther than the renal veins, the proposed filter was implanted, the size of which was chosen according to the size of the vein (the applicant called his own filter KaBaфильТР "KopoHa" No. 2).

[0089] Although the inferior vein is obviously deformed, the filter is installed in the gap of the vein strictly along the axis of the vein due...

example 2

[0090] Example 2. Female patient Р, 36 years old, developed a right-sided iliofemoral thrombus with spread to the level of the inguinal fold against the background of diffuse chemotherapy for breast cancer. There were no signs of pulmonary thrombosis.

[0091] Considering the necessity of performing repeated chemotherapy procedures, thus deepening the flow of the thrombus, Patient P had the proposed filter implanted in the typical location—the extrarenal portion of the inferior vena cava. Four months after a chemotherapy program, the patient was found to have intermittent thrombus on the filter, with transient collapse. A thrombus in the shape of a giant thrombus within the filter can be evidenced by a degenerative inferior hologram.

example 3

[0092] Example 3. Male patient л, 73 years old, implanted in the inferior vena cava due to deep vein thrombosis of the left lower extremity, another proposed filter structure "sand timer". However, three months later the patient developed a pulmonary artery thrombosis. The study found that the origin of the thrombus was a floating thrombus of the inferior vena cava, which spread above the filter, but missed the level of the confluence of the inferior vena cava of the renal vein by 1 cm.

[0093] The proposed filter KaBaфильТР "KopoHa" was successfully implanted in the athrombic segment of the extrarenal part of the vena cava with the structural features of the proposed component for the prevention of pulmonary thrombosis.

[0094] During the long-term observation of the patient, no recurrence of pulmonary thrombosis was found.

Abstract

The group of useful inventions concerns the field of medical technology, more specially to surgical instruments for catching of thromoemboli and to devices for their transport to an installation site. The a set for preventive maintenance of a thromboembolism of a pulmonary artery comprises a filter and a mounting device for its installation and extraction. In the mounting device for the installment of the filter the pusher (3) is provided with a movement limiting stop (4), and its working block is made in the form of a capsule (5) with an axially movable therein clamping cartridge (6) and a mechanism for its movement. The working block (10) of the extractor device is made in the form of a casing (13) with an aperture and a loop extending therefrom in perpendicular direction to the axis of the extractor. In the filter, the free ends (19, 20) of the fixing and centering elements are bent outwards. The threads (23) of both elements have an identical radius of bend at the end (18). The free ends (20) of the centering element have blunted ends and are made in the form of a circular-arch segment preventing the blunted ends of the fully unfolded centering legs from touching the walls of the vena cava and being directed to the centre of the lumen of the vein. The free ends (19) of the fixing element (17) are provided with protective areas (20) located between the conical part and the bent outwards fastener part and in parallel to the axis of the vein.

Description

technical field [0001] This group of inventions relates to the field of medical technology, namely to surgical tools for capturing thrombus and devices for delivering the tool to the installation site, in particular to kits and intravenous filters for preventing pulmonary artery thrombosis. Background technique [0002] According to the technical essence, the closest to the technical solution of the present invention is a complete set of components for preventing pulmonary artery thrombosis, including a filter, a device for installing the filter into the vena cava, and a device for removing the filter from the vena cava. The filter consists of a main body, a centered and fixed filter conical element mounted therein with a joint top and a leading edge, and the filter consists of divergent flexible threads distributed uniformly with each other along the circumference, said The device for installing the filter into the vena cava includes a cannula-catheter, an installation cann...
