Problems solved by technology

Because serum creatinine is a marker of filtration rate, it is not possible to distinguish between causes of AKI (prorenal, nephrogenic, postrenal obstruction, atherosclerotic, etc.) or nephrogenic disease (eg, renal in origin tubule, glomerulus, or renal space) type or location

[0104] Preparation of solid phase and detectably labeled conjugates typically involves the use of chemical crosslinkers. Crosslinkers contain at least two reactive groups, and are generally classified into homofunctional crosslinkers (containing identical reactive groups) and heterofunctional crosslinkers (containing non-identical reactive groups). Homobifunctional crosslinkers that couple through amines, thiols, or react nonspecifically are available from a number of commercial sources. Maleimides, alkyl and aryl halides, α-haloacyl and pyridyl disulfides are thiol reactive groups. Maleimides, alkyl and aryl halides, and α-haloacyls react with thiols to form thioether linkages, while pyridyl disulfides react with sulfhydryls to produce mixed disulfides. The pyridyl disulfide product is cleavable. Imide esters are also very useful for protein-protein crosslinking. A variety of heterobifunctional crosslinkers, each combining different properties for successful conjugation, a...

Embodiment 1

[0143] Example 1: Sample collection of contrast agent-induced nephropathy

[0144] The purpose of this sample collection study was to collect plasma and urine samples as well as clinical data from patients before and after receiving intravascular contrast media. Approximately 250 adults undergoing radiographic / angiographic procedures involving intravascular administration of iodinated contrast agents were recruited. To be recruited into this study, each patient must meet all of the following inclusion criteria and not meet any of the following exclusion criteria:

[0145] Inclusion criteria

[0146] Men and women aged 18 or over;

[0147] Undergoing a radiographic / angiographic procedure involving intravascular administration of a contrast agent (eg, CT scan or coronary intervention)

[0148] Hospitalization is expected for at least 48 hours after contrast administration.

[0149] Able and willing to provide written informed consent to participate in the study and follow a...

Embodiment 2

[0160] Embodiment 2: cardiac surgery sample collection

[0161] The purpose of this sample collection study was to collect plasma and urine samples and clinical data from patients before and after undergoing cardiovascular surgery, a procedure known to be potentially damaging to renal function. Approximately 900 adults undergoing such procedures were recruited. To be recruited into this study, each patient must meet all of the following inclusion criteria and not meet any of the following exclusion criteria:

[0162] Inclusion criteria

[0163] Men and women aged 18 or over;

[0164] undergone cardiovascular surgery;

[0165] A Toronto / Ottawa Predictive Risk Index for Renal Replacement risk score of at least 2 for renal replacement risk (Wijeysundera et al., JAMA 297:1801-9, 2007); and

[0166] Able and willing to provide written informed consent to participate in the study and follow all study procedures.

[0167] exclusion criteria

[0168] known pregnancy;

[0169]...

Abstract

The invention relates to methods and compositions for diagnosis and prognosis of renal injury and renal failure. The present invention relates to methods and compositions for monitoring, diagnosis, prognosis, and determination of treatment regimens in subjects suffering from or suspected of having a renal injury. In particular, the invention relates to using assays that detect one or more of Insulin-like growth factor-binding protein 2, Insulin-like growth factor-binding protein 3, Insulin-like growth factor-binding protein 4, and Insulin like growth factor-binding protein 6 as diagnostic andprognostic biomarker assays in renal injuries.

Description

[0001] This invention claims from U.S. Provisional Patent Application No. 62 / 160,519, filed May 12, 2015; from U.S. Provisional Patent Application No. 62 / 160,523, filed May 12, 2015; U.S. Provisional Patent Application No. 62 / 160,526 and priority from U.S. Provisional Patent Application No. 62 / 160,531 filed May 12, 2015, each of which is incorporated by reference in its entirety (including all tables , drawings and claims) are incorporated herein. Background of the invention [0002] The following discussion of the background of the invention is provided merely to assist the reader in understanding the invention and is not admitted to describe or constitute prior art to the invention. [0003] The kidneys are responsible for the excretion of water and solutes from the body. Its functions include maintaining acid-base balance, regulating electrolyte concentrations, controlling blood volume, and regulating blood pressure. Thus, loss of renal function through injury and / or dise...
